Hey, I made this remix, but it seems to have gotten someone else's name on it for some reason. I'm was sure that I picked out my name in the list of arrangers when I submitted, but I apparently didn't! I also noticed that the name of the song was altered by whoever accepts submissions at RKO.

Anyone know the guy who does that?

Maybe you did that mistake and Jan changed it kindly.
Concerning the wrong arranger I would suggest to contact Jan through the contact form at RKO.
